This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.
Your Rights
When it comes to your health information, you have certain rights. This section explains your rights and some of our responsibilities to help you.
Get an electronic or paper copy of your medical record
- You can ask to receive an electronic or paper copy of your medical record and other health information we have about you. Ask us how to do this.
- We will provide a copy or a summary of your health information, usually within 30 days of your written request. We may charge an actual cost-based fee for each request including the Therapist’s fee for time spent on a request.
You may be denied access to any “Psychotherapy Notes” if we believe that a limitation of access is necessary to protect you from a substantial risk of imminent psychological impairment or to protect you or another individual from a substantial risk of imminent and serious physical injury.
Ask us to correct your medical record
- You can ask us to correct personal health information about yourself that you think is incorrect or incomplete. Ask us how to do this.
- We may say “NO” to your request, but we’ll tell you why in writing within 60 days
Request confidential communications
- You can ask us to contact you in a specific way (ie; home phone rather than cell phone) or to send postal mail or email to a preferred address.
- We will say “yes” to all reasonable requests. Any request that violates another patients rights will be dismissed
Ask us to limit what we use or share
- You can ask us not to use or share certain health information for treatment, payment, or our operations. We are not required to agree to your request, and we may say “no” if it would affect your therapy or well being of another.
- If you pay for a service or health care item out-of-pocket in full, you can ask us not to share that information with your health insurer. We will comply unless a law requires us to share that information
Get a list of those with whom we’ve shared information
- You can ask for a list (accounting) of the times we’ve shared your health information for six years prior to the date you ask, who we shared it with, and why.
- We will include all the disclosures except for those about treatment, payment, and health care operations, and certain other disclosures (such as any you asked us to make). We’ll provide one accounting per year but will charge an actual cost-based fee per request including the Therapists time spent. Multiple requests may incur a postponement until final termination of your therapy or the current years end, whichever event occurs first.
Get a copy of this privacy notice
You can ask for a paper copy of this notice at any time, even if you have agreed to receive the notice electronically. We will provide you with a paper copy within a reasonable time frame
Choose someone to act for you
- If you have given someone medical power of attorney or if someone is your legal guardian, that person can exercise your rights and make choices about your health information.
- We will make sure the person has this authority and can act for you before we take any action
File a complaint if you feel your rights are violated

Your Choices
For certain health information, you can tell us your choices about what we share. If you have a clear preference for how we share your information in the situations described below, talk to us.
In these cases, you have both the right and choice to tell us to:
- Share information with your family, close friends, or others involved in your care
- Share information in a disaster relief situation
If you are not able to tell us your preference, for example if you are unconscious, we may go ahead and share your information if we believe it is in your best interest. We may also share your information when needed to lessen a serious and imminent threat to health or safety.
In these cases we never share your information unless you give us written permission:
- Marketing purposes
- Sale of your information
- Most sharing of psychotherapy notes
- In the case of fundraising, we may contact you for fundraising efforts, but you can tell us not to contact you again.
Our Uses and Disclosures
How do we typically use or share your health information?
We typically use or share your health information in the following ways.
Treat you
We can use your health information and share it with other professionals who are treating you. Example: A doctor treating you for an injury asks another doctor about your overall health condition.
Run our organization
We can use and share your health information to run our practice, improve your care, and contact you when necessary. Example: We use health information about you to manage your treatment and services.
Bill for your services
We can use and share your health information to bill and get payment from health plans or other entities. Example: We give information about you to your health insurance plan so it will pay for your services
Help with public health and safety issues
We can share health information about you for certain situations such as:
- Preventing disease
- Helping with product recalls
- Reporting adverse reactions to medications
- Reporting suspected abuse, neglect, or domestic violence
- Preventing or reducing a serious threat to anyone’s health or safety
Do research
We can use or share your information for health research.

We will share information about you if state or federal laws require it, including with the Department of Health and Human Services if it wants to see that we’re complying with federal privacy law.
Work with a medical examiner or funeral director
We can share health information with a coroner, medical examiner, or funeral director when an individual dies.

Linked & 3rd Party Access to Information (Third Party Access)
Our office reduces the amount of 3rd party or shared communication of information with persons, entities, websites, companies or organizations outside of our office. However, some products that are used to conduct business require personal patient data (personal & financial information, images, preferences) to be shared/collected/transmitted. Nanette Sebourn LMFT is not responsible for outside parties or electronic medium’s mishandling of any information they are provided or have obtained. We will always take steps to try and prevent Linked Intrusion by a third party. Our site www.NanetteSebourn.com contains links to independent outside websites (which also post their own links). If you link to a third party site from www.NanetteSebourn.com, any information you provide a third party website is not covered by this privacy policy. To be informed of the third party privacy rules, you must read and adhere to their privacy statement.
Website Tracking (“Cookies”)
We may also collect information about you and your use of our site through the use of cookies to verify city/state/country in an effort to improve your online experience. “Cookies” are small computer files that websites transfer to your computer’s temporary internet files that provide info regarding visit information (ie: length of visit, page viewing & personal preferences).
Electronic Communications (Emails) & TeleHealth \ TeleTherapy Services
Email & other online communication is a common transmission of information. While we take care to provide secure communications via these electronic mediums, please note that email does have its drawbacks & online transmissions are susceptible to breaches in security.
We may choose to respond or engage with you in an electronic format. Replies to electronic mail will be like-kind and any requests for documentation to be transmitted via email may be subject to the insecurity inherent in the medium. While efforts are used (virus & threat scanning), Nanette Sebourn LMFT takes no responsibility regarding lost, stolen, hacked, phished, denied or otherwise compromised emailed & online communications (Including 3rd party tools/websites).
Deposits & Pre-Payments for Goods OR Services
Any prepayments, deposits, checks, money orders, cashiers checks, any “Crypo-Currancy” or other forms of legal tender offered to Nanette Sebourn LMFT through any means, mediums, intermediaries or website (including 3rd Party or “Unofficial” websites) are non-refundable & non-transferable. Any alteration to this policy is subject to additional fees & are subject to change at the discretion of Nanette Sebourn LMFT.
Audio & Video Surveillance
Nanette Sebourn LMFT or the building housing our current office may employ the use of audio & video recording equipment. All images or recordings are managed by the entity they were recorded by. Nanette Sebourn LMFT is not responsible for recordings obtained outside of our current office. Any recordings/audio samples/images/videos/media created within our office will be used at Nanette Sebourn LMFT’s discretion & may/may not be used in therapy to further assist out patients. By entering our office location or interacting online with Nanette Sebourn LMFT you consent to audio & video surveillance or recordings during your visit and/or therapy session with the Therapist.
Voluntarily Submitted Information
In addition, we collect and retain information you voluntarily submit to us. It is up to you whether to submit information to us, and how much information to provide. We may ask for identifying information such as, but not limited to, your name, email address, mailing address and phone number and will retain any & all information deemed important or relevant.
COVID-19 & Social Protocol
Our office, while sanitary & cleaned on a normal basis, has increased the frequency & detail of our cleaning regiment. However, all patients are responsible for their own physical health. Please inform this office if any symptoms of the COVID-19 virus are present. We reserve the right to modify the procedure in which we address each individual case/occurrence based on prevailing information at hand. The rapid changes in advice/knowledge/best practice regarding this most current virus have demanded ad-hock procedures in guarding against the spread. It is up to you, the patient, to wear any personal protective equipment (PPE) deemed acceptable to prevent the contraction/spread of any disease, including COVID-19.

All recommendations made by Nanette Sebourn LMFT and its agents (including published media) are based upon opinions and/or observations. Nanette Sebourn LMFT and its agent’s suggestions are not intended to diagnose, treat, cure or prevent any disease.
